      Are there any services, procedures or concerns that need to be addressed during their stay with us? Please check the appropriate box and specify in the box provided.
      Are we able to refill any medications during their stay? Please check the appropriate box and specify in the box provided.
      Would you like a refreshing discounted bath before being discharged from boarding?
      Due to the fact that we are primarily a veterinary medical care facility, we have very young and very old guests, as well as chemotherapy and other immune compromized patients. Therefore, we require the following vaccines/tests be up to date for all of our guests:
      Dogs: Rabies, DHPP (Distemper), Leptospirosis, Tracheobronchitis (Bordetella, Kennel Cough), Canine Influenza (CIV), and a negative test for intestinal parasites and heartworm within a year.
      Cats: FVRCP (Distemper), Rabies and a negative test for intestinal parasites

      Should vaccinations/tests are not up to date by the time of arrival, they can be done during their stay. Please contact our office with any questions regarding requirements and timing.
